if the applicant is a subsidiary of the parent company you must provide the entire ownership chain up to the ultimate owner
Balance Sheet and Income Statement (profit/loss) dated within the last 6 months
Balance Sheet and Income Statement (profit/loss) dated within the last 6 months for businesses that are members of the applicant (LLCs only)
If the applicant's capital stock is owned in whole or in part by a parent company, a Balance Sheet and Income Statement (profit/loss) dated within the last 6 months of the parent company
Schedule Listing of any person whose stockholdings are held in a name other than the stockerholder's real name with address, holding, and beneficial owner of stock (Corporations only)
Proof of registration with the Delaware Divison of Corporations
Schedule of Details if answering yes to questions 13, 14, or 15
Photocopies of legal documents pertaining to the matter if answering yes to question 15
Affidavit of individuals who questions 13, 14, or 15 are applicable if the applicant can't swear to the truth on their behalf
Detailed Business Plan describing activities of business in which applicant intends to engage
List all products offered and attach copies of letters from the Banking Commissioner authorizing the listing of the products, if applicable
List of other states where applicant is transacting business and nature of business transacted
Submit name of regulatory agency, address, telephone number, contact name, and email for each state listed
3 letters of reference from businesses currently doing business with the applicant company (one must be a bank)
These references must be sent directly to the Office of the State Bank Commissioner via US Mail
Submit copies of requesting these references with the application
