Gaming Service Provider Registration form

Please contact the Bureau of Licensing for electronic submission instructions once the application is filled out.

$500 application fee + $60/affiliated entity + $60/qualifier + $350/gaming employee permit + $60/non-gaming employee registration + $2,500 registration fee

• Current ownership and management tables of the organization
• If the applicant has been placed on the Commonwealth Debatement List maintained by the Department of General Services, please provide a list of the dates of any debatement and an explanation for the debatement
• If your enterprise is providing transportation services (directly or indirectly through a transportation company) supply copies of the enterprise's Certificates of Registration or documentation from any government agency that allows the enterprise to provide transportation service
• Verification Letter from the Bureau of Small Business Opportunities and/or Small Business Procurement Initiative certification which identifies the company as a small diverse business (if applicable)

$500 application fee + $60/affiliated entity + $60/qualifier + $200/gaming employee permit + $40/non-gaming employee registration + $2,500 registration fee

Every 5 years by date of issuance

An analyst will be assigned to each registrant after they submit their initial application. When the license nears its expiration, the analyst will be in contact with the registrant regarding the renewal process, required forms/documents, processes, etc.
