Instructions:	Vendors seeking to apply for vendor registration must contact the Casino they plan to work with directly to obtain a Vendor Registration Form and a Vendor Registration Supplemental Disclosure Form. The Vendor Registration Form is submitted to the Casino who then files it with the Division. Within 30 days of submitting the Vendor Registration Form to the Casino, the applicant must submit the Vendor Registration Supplemental Disclosure Form to the Division.

Not required

Vendor Registrations are not required to be renewed and will remain active unless revoked, suspended, limited or otherwise restricted by the Division.
If a Vendor does not conduct business with a casino hotel facility for a consecutive period of 3 years, the registration of that vendor will lapse.
