Agency Fee:
  • $100 wall license fee + $257.50 for one set of vehicle registrations and five license plates (due upon preliminary approval)
Required Attachments:
  • Surety Bond (due upon preliminary approval)
  • Certificate of Insurance (due upon preliminary approval)
  • Fingerprint Request Notification
  • Child Support Form BLS-43 (must be completed even if you do not have these obligations)
  • Copy of driver's license for each owner, partner, member, and officer listed on the application
    • Nonresidents must provide 6 points of identification. Information regarding this requirement can be found here
  • Passport sizes color photograph for each owner, partner, member, and officer listed on the application (print name on back of each photo)
  • Copy of Formation Filing with the New Jersey Division of Revenue (must show filing date)
  • Copy of Alternate/Fictitious Name Filing Certificate, if applicable
  • Copy of FEIN registration certificate
  • Copy of Property Deed, Lease, or Rental Agreement
  • Copy of Certificate of Authorization (foreign corporations and LLCs only)
  • Copy of New Jersey Sales Tax Certificate of Authority
  • Copy of Franchise Agreement (new dealers only)
  • Certified statement that facility is not less than 1,000 square feet (new dealers only)
  • Business Hours Form
  • Completed Authorized signatories Form BLS-20 (employees must provide copies verifying employment)
  • Municipal Approval Certificate for Business License
  • Dealer Certification of License Location Type and Proper Walls form BLS-158
  • Copy of telephone bill for landline telephone with the business name and address
  • Photographs/plans clearly depicting complete premises and signage where dealer intends to conduct business
