Insurance Requirements:	Certificate of Worker's Compensation Insurance or Valid Exemption form. The requirements are as follows: Policies issued outside the State of Florida must identify proof of coverage for the State of Florida in the "Description of Operations" box. The company and individual names listed on the certificate must match the corresponding State Licenses. License numbers of individuals qualifying a company must be listed in the "Description of Operations" box of the certificate. Other sections must read as: Producer: upper left corner of ACORD form must include the insurance agency name, address, and phone number Certificate holder: lower left corner of ACORD form must read Division of Building Safety Contractor Licensing Post Office Box 2687 Orlando, Florida 32802 Authorized Representative: lower right corner of ACORD form must include the agents' individual signature

Form:	There is no form to renew a State-Certified Contractor Registration in Orange County. Mail or e-mail updated copies of the required documents to the Division of Building Safety to keep a registration current.
