  • $200 registration fee unless a MWBE
  • $100 for MWBEs
Required Attachments:
  • Outstanding Wage Assessments
  • Federal and State Debarment History over the last 8 years
  • Final Determinations of violations related to any of the following:
    • Labor Laws
    • Tax Laws
    • Workplace Safety Laws and Standards
  • Proof of participation in a New York Apprenticeship Program, if applicable
  • Proof of Worker's Compensation or Valid Exemption Form
  • Proof of status as a New York Certified MWBE, if applicable

Registration Renewal

Filing Method:

Online

Agency Fee:
  • $200 registration fee unless a MWBE
  • $100 for MWBEs
Due:

Registrations expire annually by the date of issuance. Renewals must be completed at least 90 days prior to the expiration of the license. 

Required Attachments:
  • Outstanding Wage Assessments
  • Final Determinations of violations related to any of the following:
    • Labor Laws
    • Tax Laws
    • Workplace Safety Laws and Standards
  • Proof of Worker's Compensation or Valid Exemption Form
  • Proof of status as a New York Certified MWBE, if applicable
