Before you Apply:
  • Businesses who think they need a Sports Wagering Registration must complete the Request for Determination on Sports Wagering Registration
    • Marketing affiliates, data centers, and payment processors do not need to submit this form, they can immediately submit the registration form and application fee
Required Attachments:
  • Confirmation of Business Relationships from Operator
  • Certificate of Good Standing from Massachusetts Department of Revenue or a letter indicating the entity is not a business with Massachusetts taxable income and is not filing other Massachusetts taxes on a regular basis
  • Articles of Incorporation
  • Diversity Certification, if applicable
