Insurance Requirements:	General Liability Insurance in a minimum amount of $100,000. If the insurance company is out of state it must say the policy is good in the state of Louisiana. The sole certificate holder must be the City of Sulphur, P.O. Box 1309, Sulphur, LA 70664. For contractors doing work in the City of Sulphur, the minimum General Liability Insurance is $300,000.
Bond Requirements:	$15,000 surety bond made favorable to the City of Sulphur stating the trade classification in which you are operating
