Insurance Requirements:	Certificate of Insurance with the following: General Liability: each occurence = minimum of 500,000 General Liability: general aggregate = minimum of $1,000,000 Workers Compensation & Employer's Liability: EA accident = minimum of $100,000 Workers Compensation & Employer's Liability: disease EA employee = $100,000 Workers Compensation & Employer's Liability: disease policy limit = $500,000 If you are the sole employee you can apply to the state of Indianna Department of Revenue for the worker's compensation waiver.
Bond Requirements:	Minimum of $5,000 recorded with the Lake County, Indiana Office of the Recorder. The following verbage must be included on the bond in its entirety, "The board of commissioners of the county of Lake, state of INdianna, and all cities, towns and municpalities within".
Registered Agent (Special Agency) Required?	No
Requirement Notes:	The Contractor's company information must match exactly on both the Certificate of Insurance and the Lake County Bond.
