Notes:	Upon receipt of this completed form, the municipality will provide any additional forms and information regarding other specific requirements in order to complete the licensing process Any business outside the municipality but within the police jurisdiction will only be required to pay 1/2 the amount of the license fee imposed

Penalties:	Renewal applications not submitted by January 31 will be considered delinquent except for insurance companies, which will be delinquent after March 1
